Practice Location
Parchment Family Practice, P.C. is located at 2350 East G Avenue in Parchment, MI 49004. Office hours are Monday from 8:00 a.m. to 6:00 p.m., Tuesday through Friday 8:00 a.m. to 5:00 p.m. Additional hours may be used to accommodate patients.

General Policies
Patients are seen by appointment only. For emergencies, please call before coming to the office.
Please try to call several weeks in advance for routine appointments and physicals. For more urgent matters, our staff will try to get you in on the same day.
The phone is answered 24 hours a day. If Dr. Born is unavailable, there will be another physician "On-Call." After hours calls are for emergency and urgent situations.
The Patient/Provider Partnership
The health and wellness of our patients is our primary concern in this office. Providing the best possible care to every patient is our goal. The only way we can meet this goal is if the patient and provider work together. This concept is called the Patient Centered Medical Home.
Patient Responsibilities
- Ask questions, share your feelings, and be part of your care.
- Be honest and accurate about your history, symptoms, and other important information about your health.
- Inform your doctor about any changes in your health and well-being.
- Take all of your medicine and follow your doctor's advice.
- Make healthy decisions about your daily habits and lifestyle.
- Prepare for and keep scheduled visits or reschedule visits in advance whenever possible.
- Call your doctor first with all problems, unless it is a medical emergency.
- End every visit with a clear understanding of your doctor's expectations, treatment goals and future plan.
Doctor Responsibilities
- Explain diseases, treatments, and results in an easy-to-understand way.
- Listen to our patient's feelings and questions to help them make decisions about their care.
- Keep treatments, discussions, and record confidential.
- Provide 24 hour access to medical care and same day appointments for urgent care needs, whenever possible.
- Provide instructions on how to meet your health care needs when the office is not open.
- Care for you to the best of our abilities based on our understanding of current medical alternatives available.
- Give our patients clear directions about medication and other treatments.
- Send our patients to trusted specialists, if required.
- End every visit with clear instructions about expectations, treatment goals and future plans.
Confidentiality and Privacy
Confidentiality and privacy of your health and financial information is of ultimate priority to us. Our staff will ensure that your information is maintained and disclosed only in the proper manner and in accordance with the Health Insurance Portability and Accountability Act (HIPAA) guidelines.
